WooHoo Ladies of the canyon: Jamestown Area Community Foundation (JACF), is bridging together opportunities with organizations to sustain health and wellness here in the canyon! This year we are proud to offer the opportunity for 12 women to receive a mobile mammogram from Saint Joseph Hospital(Denver) Mobile Mammography Unit *see video attached. (unfortunately 12 participants are all they're able to offer at one time-should we see the desire for more appointments, then next year we'll be able to host multiple days).
Important Info:
When: Mon 8/22/2022 By Appt. 10AM-2:40PM (every 20 min)
Where: Parking lot of little park (Across from Townhall)
Cost: Accepts most private insurances (EXCEPT KAISER) Accepts Medicare & Medicaid & self pay rate
Self pay rate: $166 (for mammogram) + ~$150 (for radiologist read) TOTAL self-pay =~$316

At appt. - make sure you have ins card and driver's license you'll be asked to fill out the intake form OR you can fill it out (attached) and print it prior to the appt. to bring with you.
